Personal Information

The personal information page consists of additional information that helps identify you. This information is not required, but may be helpful to someone acting on your behave. For example, the medical information may be critical if you are hospitalized and unable to communicate.

You can press the F1 key when focused on any particular field for more specific help.

Field Information

Field	Description
Madien Name	Your Madien Name if appropriate.
Birth Place	The country or state in which you were born.
Nationality	Your country of origin or legal residence.
Race	Your legal ethnic Race.
Religion	Your practiced or prescribed religon if appropriate.
Donor	Identifies you as an organ donor.
Blood Type	Identifies your blood type.
Medical General Information	Any medical or general information you wish to include with your letter. An example would be to identify yourself as a diabetic if medical treatment is required.
